- Endocrinologist
- Create a personalized treatment plan that includes medication management
- Offer screening, diagnosis and management of Diabetes,
- Set target goals and results
- Referral team including Podiatrist, Ophthalmologist, Dentist, Psychiatrist, Cardiologist, Neurologist, Nephrologist, Sports medicine
- To manage and support Diabetes associated risk factors and complications
- Dietitian: Provide nutritional guidance to make healthier food choices
- Diabetes Educator:
- Provide support by holding individual education sessions on healthier lifestyle changes, glucose monitoring, Insulin administration.
- Perform testing of autonomic peripheral neuropathy using Sudo scan device
- Provide assistance in application of sensor for continuous glucose monitoring
